K Health is seeking a dedicated Primary Care Physician to join our team and provide comprehensive, fully remote clinical care. By partnering with top-rated hospital systems, we utilize a cutting-edge Clinical AI Platform to expand access to primary care for patients facing barriers to in-person visits, ensuring high-quality preventive, acute, and chronic condition management.

Key responsibilities

• Deliver high-quality preventative, acute, and chronic care in a fully remote, virtual environment.
• Manage a dedicated panel of patients, providing compassionate and meaningful clinical encounters.
• Refer patients to specialists or acute care services when clinically indicated.
• Support patients post-discharge to improve treatment plan adherence and reduce readmissions.
• Engage in population health initiatives to close care gaps and improve patient outcomes.

Requirements

• Board Certification in Family or Internal Medicine.
• At least 3 years of post-residency experience, including 2+ years in an in-person primary care setting.
• Active North Carolina state medical license.
• Prior experience with EPIC EMR and virtual visit platforms.
• Ability to commit to a 40-hour work week with specific availability on Mondays and Fridays.
