Quality Improvement Specialist

Evergreen Nephrology is transforming kidney care through a value-based, person-centered approach. We are seeking a dedicated Quality Improvement Specialist to join our team and help improve patient outcomes by bridging the gap between providers and patients.

Key responsibilities

• Serve as a trusted partner to nephrology practices, interpreting quality reports and dashboards to drive performance improvements in HEDIS and Stars measures.
• Manage an assigned patient panel, conducting proactive outreach to support patients in completing screenings, labs, and recommended care.
• Educate provider practices on documentation accuracy, coding completeness, and best-practice workflows.
• Collaborate with cross-functional teams, including Clinical Operations and Pharmacy, to ensure aligned support for patients and providers.
• Maintain accurate documentation in EMRs and quality systems while adhering to HIPAA and regulatory compliance standards.

Requirements

• 1-2 years of experience in quality improvement, population health, or care coordination.
• Proven experience in patient-facing outreach and relationship building.
• Strong understanding of quality workflows, gap closure processes, and healthcare documentation requirements.
• Proficiency with EMR systems and MS Office Suite.
• Ability to work effectively in a remote environment with a dedicated, secure workspace.
