Shields Health Solutions is redefining patient care through health-system integrated specialty pharmacy. We are seeking an Ambulatory Oncology Clinical Pharmacy Specialist to play a hands-on role in elevating patient care at a nationally recognized Academic Medical Center in a fully remote capacity.
Key responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards.
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement and promote seamless medication coordination.
- Monitor therapy effectiveness and safety, identify optimization opportunities, and intervene as appropriate.
- Support quality-improvement and REMS initiatives while providing recommendations to operations regarding complex prior authorizations and clinical workflows.
Requirements
- Doctor of Pharmacy (PharmD) degree and active pharmacist license in California.
- Completion of a PGY2 Oncology Residency, or PGY1 Ambulatory Care Residency with at least 3 years of clinical oncology experience.
- Alternatively, at least 3 years of experience as a clinical pharmacist in oncology, including 1 year practicing under a Collaborative Practice Agreement.
- Strong communication, organization, and technical skills combined with an empathetic, team-first mindset.
