Clinical Pharmacy Specialist

Shields Health Solutions is redefining patient care through health-system integrated specialty pharmacy. We are seeking an Ambulatory Clinical Pharmacy Specialist to play a hands-on role in elevating patient care at a nationally recognized Academic Medical Center in a fully remote capacity.

Key responsibilities

• Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards.
• Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement and promote seamless medication coordination.
• Monitor therapy effectiveness and safety, identify optimization opportunities, and intervene as appropriate.
• Support quality-improvement and REMS initiatives.
• Provide recommendations to operations by supporting teammates with complex prior authorizations, improving workflows, and educating patients.

Requirements

• Doctor of Pharmacy (PharmD) degree.
• Active pharmacist license in California.
• Completion of a PGY2 Ambulatory Care Residency, OR a PGY1 Residency with at least 1 year of ambulatory practice under a CPA, OR at least 3 years of ambulatory practice with disease state management.
• Strong communication, organization, and technical skills.
